How to fill this form

Step 1 — Complete Sections A–B first. If any required eligibility item is "No", stop and mark the record as Not Eligible.
Step 2 — Score each domain using whole numbers only (0–5). Use the anchors to choose a consistent score.
Step 3 — Provide brief, objective justification. 2–4 sentences focusing on the manuscript (avoid personal remarks).
