ACAM — Awards Evaluation Form Internal Use Only

Structured scoring for ACAM Prizes & Awards (AMA / ICMJE / COPE / WAME aligned) with reporting guidance (CONSORT / PRISMA / STROBE / CARE).

How to fill this form

Step 1 — Complete Sections A–B first. If any required eligibility item is "No", stop and mark the record as Not Eligible.
Step 2 — Score each domain using whole numbers only (0–5). Use the anchors to choose a consistent score.
Step 3 — Provide brief, objective justification. 2–4 sentences focusing on the manuscript (avoid personal remarks).
Step 4 — Declare conflicts of interest. If conflicted, select Recused and do not score.
Guidance: This is an awards evaluation form (not a peer-review report). It must remain confidential and should not be shared with authors.
